[classified.csv] fixed some rows #url not replaced (#154)

Fixes: jonathanpeppers/inclusive-code-reviews-browser#290

I found a row with the text `183` that likely caused this.

I found places that URLs were not replaced with `#url`, and also duplicate rows.

I resorted alphabetically & removed duplicates after my changes.
