It seems only substreams build takes it into consideration:

$ substreams protogen
Running: buf generate /var/folders/hy/f_lkgxf91_b7mt3trmxw671w0000gp/T/solana_tokens.tmp.spkg#format=bin

$ substreams build
Running: buf generate /var/folders/hy/f_lkgxf91_b7mt3trmxw671w0000gp/T/solana_tokens.tmp.spkg#format=bin --exclude-path sf/substreams
...
