If you use a mod that replaces the Nether Fortress structure it seems like the Nether Keeper will never spawn (at least from my testing). Might be worth including other valid structures for the mob to spawn in.

In 1.19.2+ there is already compatibility...(using tags)
Backport to 1.18.2 is not active, sorry.
