Create Project :: Add ability to select which fields get brought into Cadasta

Users may not wish to upload all of the columns in the layer that they have in QGIS.

In the "Create Project" series of steps (after mapping the required fields), there should be a table view or list view where users can toggle on or off the column to be included in the questionnaire.
